Eligibility for tax relief in any given year is based in part on your prior year's household income. If you filed a federal income tax return, a copy of the return, including all schedules and attachments, must be included with your application for relief.
If you did not file a federal income tax return, then you must include copies of all 1099's issued by financial institutions and other payers (including Social Security Benefit statement).
You may also be asked to provide copies of year-end bank statements or any other documentation necessary to determine eligibility.
